Effects of Bariatric Surgery on Reproductive, Metabolic, and Gynaecological Outcomes in Women with Polycystic Ovary Syndrome: A Systematic Review and Meta-analysis

Objectives:
To evaluate the effects of bariatric surgery on reproductive, metabolic and gynaecological outcomes in women with PCOS compared with non-surgical management. To compare outcomes across different bariatric procedures and assess fertility-related outcomes (ovulation, conception, live birth, miscarriage). To evaluate metabolic improvements, changes in menstrual regularity and hyperandrogenism, and adverse events associated with bariatric surgery.
Study Overview:
Polycystic Ovary Syndrome (PCOS) is a common endocrine disorder among women of reproductive age and is frequently associated with obesity, insulin resistance, metabolic disturbances and adverse reproductive outcomes. This systematic review and meta-analysis aim to evaluate the effects of bariatric surgery on reproductive, metabolic and gynaecological outcomes in women with PCOS compared with non-surgical management.
Study Approach:
A systematic review and meta-analysis will be conducted following a predefined protocol. Studies involving women diagnosed with PCOS undergoing bariatric surgery will be included, with comparators including lifestyle interventions and medical management such as anti-obesity medications. Eligible studies will be identified through comprehensive database searches and data will be synthesised using meta-analytic methods where appropriate.
Expected Public Health Impact:
The study will generate consolidated evidence on the effectiveness and safety of bariatric surgery in women with PCOS. The findings will support evidence-based clinical decision-making and may inform clinical guidelines and policy discussions related to obesity management and reproductive health in India. The database search has been completed and screening of titles and abstracts is currently ongoing using the Rayyan platform.
